What is ICT?

The Refugee Health Integrated Care Team is a collaborative team designed to provide support and resources to Primary Care Provider’s that accept newcomer refugee patients into their practice to help make the transition of care smoother.

A collaboration of organizations including The Centre for Family Medicine, Community Healthcaring Kitchener-Waterloo, Camino Wellbeing + Mental Health, Home and Community Care Support Services, Reception House, and YMCA Settlement Services.

  • Accommodate the diverse health, social, and cultural needs of newcomers
  • Encourage and support primary care practices to accept more newcomers

  • Counselling services- Camino (formerly Carizon)
  • Refugee Care Coordinator (Home and Community Care Support Services)
  • ICT Coordinators- Support patients and providers in whatever way they need
  • Case manager supports- Reception House
  • Outreach supports- YMCA
