About Us

The Centre for Family Medicine (CFFM) FHT is home to the first pharmacist-directed, physician supported Point-of-Care Anticoagulation Clinic in Ontario.

The Anticoagulation Clinic operates EVERY OTHER Monday from 08:00 to 11:30 at 25 Joseph Street in Kitchener; every Thursday from 8:00 to 11:30 at 25 Joseph Street in Kitchener, and every other Wednesday from 13:00 to 14:30 in Wellesley.

Patients of the physicians of the Family Health Team who are taking warfarin can drop by during these times for an INR test and an assessment by the pharmacist and nurse who run the clinic – no appointment is necessary.

Research performed at the CFFM FHT has demonstrated that the Point-of-Care Anticoagulation clinic has been associated with an improvement in patient satisfaction and the quality of anticoagulation care provided.

The CFFM FHT, in partnership with the School of Pharmacy at the University of Waterloo, offers a Continuing Professional Development course in the Management of Oral Anticoagulation Therapy for pharmacists, nurses, and nurse practitioners.  Information about this program can be found at: http://ce.uwaterloo.ca/PharmacyOverview.htm.
