Dr. Moore Practice Update - Centre for Family Medicine

Due to illness, Dr. Moore’s family practice will be closing effective August 28, 2025. All patients will receive an email or mailed letter by early August with information for the transfer of your care.

You have options, as there are some places accepting patients. Please read the full details in the letter, as they are listed for you. You may always put your name on Health Care Connect waitlist, which helps connect patients to doctors accepting patients at 1-888-579-6707, Monday to Friday, between 8:30 a.m. and 4:30 p.m.

Our Office

Please note: Dr. Jones and Dr. Clarke are not taking patients. We ask you kindly not to call the office to request that you be transferred to them. The answer will be “no” without exception.

Follow Up

If you have any outstanding tests, referrals or forms, these will be dealt with by our office between now and August 28^th. We ask for your patience in getting back to you about these items. If you are scheduled for an upcoming test, please ask for your new provider to be added to the the requisition so that they can get your results.

Current Appointments with Allied Health at the Family Health Team

If you are receiving counselling through the Centre for Family Medicine FHT, we will honour these pre-booked appointments until your therapy course is finished. If you have appointments with the MINT memory clinic or a dietitian or respiratory therapist already booked, you are welcome to attend them.

Prescriptions

Prescriptions will be renewed up until August 28^th. Please give us 2 weeks of notice for prescription refills. With the volume of requests that we anticipate, the usual 72 hours of notice is not enough. There will be no charge for faxed prescription renewals.

Coverage Until August 28^th

For most days between now and August 28^th, there will be an in-office physician covering for me. If you have an urgent issue (chest/ear or other infection, workplace injury) please call our office and we will find you the soonest possible appointment with a replacement physician, Physician Assistant or Nurse Practitioner.
